The discount medical benefits through Lifeguard Benefit Services are NOT insurance
However, your plan does include certain limited insured benefits. The plan is not a health insurance policy nor is it intended to replace or be a substitute for any health insurance. The plan provides savings at certain health care providers for medical services, but does not make any payments directly to the providers of medical services. The member is obligated to pay for all health care services, but will receive a discount from those health care providers who have contracted with the network. The range of discounts for medical or ancillary services provided under the plan will vary depending upon type of provider and medical or ancillary services rendered. The Pharmacy discounts are only available at participating pharmacies.
Discounts on professional services are not available where prohibited by law. The plans through Lifeguard Benefit Services are primarily a referral and discount savings plan, and does not warrant any professional services, nor is it responsible for the quality of care received. This plan makes no warranties, express or implied, concerning services or care provided. Savings will be derived from the discounts received from the provider’s published fees that may change from time to time. All fees and prices are subject to change at any time without notice to the member.
This is not an insurance policy and is not protected by any state Life and Health Guarantee Association. The member must be at least 18 years of age and the plan may only be used in the United States of America unless otherwise noted. A member can only be a member of any one Consumer Assistance Services Association plan at any one time. Consumer Assistance Services Association has the right to refuse membership to any person for any just or legal cause.
The member shall receive a full refund of membership fees, excluding any one-time registration fees, where permitted, if membership is canceled within the first 30 days of purchase. Members must be at least eighteen (18) years of age and reside in the United States.
Consumer Assistance Services Association may assign its duties and responsibilities to third parties, and therefore be relieved of any further liability.
