of a member’s personal information are of utmost importance. Any personal and account
data maintained by us is done to fulfill our legal and regulatory requirements and
to provide the member with the products and services requested. Personal information
includes, but is not limited to, the following: a member’s name and address, date
of birth, gender, telephone numbers, social security number, bank account or credit
card numbers, and beneficiary information. The same or similar personal information
may be maintained for a member’s named dependents. We protect a member’s personal
information by our physical, electronic and procedural safeguards.
We may use or disclose personal information to communicate the availability of health-related
benefits and services, to determine eligibility for plan benefits, to re-price medical
bills, to process benefit related claims, and to administer necessary activities
relating to the membership. Service providers that, in the normal course of business,
assist us in meeting the needs of our members may receive personal information in
order to perform the required business task. This personal information is treated
with the same standards of confidentiality that we use. We will not sell, share,
or disclose any personal information collected to any source not affiliated with
us; however, we do reserve the right to disclose or report personal information
in the following circumstances: in dealing with individuals who are involved in
your care or the payment of your care; where we believe in good faith that disclosure
is required by law; in cooperating with regulators or law enforcement authorities;
in performing necessary credit checks or collecting/reporting debts owed to us;
or in protecting our rights or property.
Our employees will be provided with relevant information in order to respond to
a member’s needs and provide a member with information and assistance. Our employees
are trained in the proper handling of member information and directed to use strict
standards of care in handling the personal, confidential information.
Because we wish to provide the member with the best possible service, the accuracy
and completeness of the member’s personal information is important to us. We attempt
to keep member information complete and up to date. Members can contact us at any
time during regular business hours to review their information and to correct or
update that information. We will maintain member information even after the member
cancels or discontinues membership. This information will be protected in the same
manner as that of an active member. A member may request in writing at any time
after discontinuing the membership to remove their personal information from our
file.
